Teacher Bio Questionnaire

We would like to include your biography and headshot with our list of faculty at Expressions! These will be displayed to our prospective and regular students when booking trial lessons and make-up lessons. They will also be displayed in our student portals. In the interest of maintaining some consistency between bios, we would like to write your bio for you using the information you provide in the questionnaire below.

Please fill out the questionnaire below with as much detail as possible. It is not possible to provide too much detail! We will make sure that your bio is consistent in length to the others.

Before submitting this form, please also email a headshot to info@expressionsmusicacademy.com. We don’t need anything glamorous and you can be shown with or without your instrument. You want to go for a professional shot from about midway or shoulders up and exude warmth and a welcoming demeanor in your facial expression. If you would like to send us multiple options, that would be great and we can choose one from there. If you feel you need help with this, let us know and we can have one of our staff take a headshot for you.
